____________ to voluntarily participate in Team Building 2019 that will be held on September
28, 2019 on 9:00AM onwards at baranggay 174 Covered Court. I ensure that my child
understand that school rules and regulations will be in effect for their safety and which should
be obeyed all the time.

I together with my child understand that participating in this kind of event may bring
unforeseeable happenings; and I hereby fuly release and discharge the Head of Committee and
