With the 100% completion of the project construction of ATM Offsite booth
at the Provincial Capitol, we are endorsing herewith the following documents
required to effect project payment, viz.:

a. Certificate of Completion
b. ATM Inspection Checklist with photos
c. One (1) set of As-built plan
d. Copy of Final Billing

In view hereof, may we request approval to effect full payment of the

aforementioned project.
