This is to certify that Mr. Nhiel Patrick Evangelista, 24 year old male, was admitted to
The Medical City General Hospital under my care last August 24, 2020. He was diagnosed as
follows:

• Poisoning due to Parcetamol

• Toxic Effects of Pyrethroid insecticides
• Major Depression considered.

He was treated for the above and his condition improved. He was discharged on August 27,
2020. He has been advised to rest at home for one week and to consult with his doctors
regularly.
