The Manager
Commonwealth Insurance Company
Cagayan De Oro City

Sir;

Good Day!

This has reference to the accident that happened to my car (Mitsubishi Strada – Plate # AAF 7829);
which car is insured with Commonwealth Insurance Company under Policy No. PCI391264.

The accident happened on May 12, 2017 however it was reported to your office very late due to the
following reasons:

1. Right after the happening of the accident, the other party (Talacag Municipality Rescue Car)
promised to pay/undertake the repair of my car as it was found out by the police authorities
that it was their fault;
2. About two weeks after the accident, the unfortunate Marawi Siege took place. We had to leave
our place and evacuated to Iligan City and later to Manila since a daughter of mine resides
there. It took us about six months before we were able to return back to Marawi;
3. Right then, we tried to the best of our efforts to contact the other party to compel them make
good the promised repair to my car. However, all our efforts were in vain since no action was
taken by them;
4. Consequently, my husband and I decided to bring the problem to the court against the other
party. However, we were advised that the process would take a long way, hence it would just
entail additional expenses on our part;
5. At this point, we decided to report the case to the Commonwealth Insurance Company, hoping
that the damage be covered despite the delay since the reasons are justifiable.

I hope this enlighten you about the case. Further, I am anticipating immediate favorable action.

Thank you very much.

Truly yours,

BOLAWAN M. GALAWAN
