Gentlemen:

With reference to the above captioned shipment, may we request your good office to allow
us to take delivery of the loaded container(s) out from the pier Zone to our warehouse
located at Mandaue City Cebu for which purpose, we hereby request you to issue in our favor a
duly accomplished Equipment Interchange Receipt (EIR) as proof of such delivery.

In exchange for the above, we hereby obligate ourselves to return the empty container(s)
within three (3) days after removal from the Pier Zone to your designated container area of
depot. Should we fail to return the same within the period stated above, we hereby agree and
bind ourselves to pay for equipment detention charges to be assessed in accordance with
your existing tariff.

Furthermore, we hold ourselves liable for any loss, destruction and or damages sustained while
the container is/are in our custody including all other cost and charges you might incur due to
loss and destruction of said equipment without further demand.

All expenses to be incurred in connection with the above transfer shall be for our account.
