Tôi xác nhận rằng thông tin cá nhân và thông tin cá nhân mang tính nhạy cảm của tôi do
tôi cung cấp là đúng và chính xác.

• I understand and agree that I am primarily responsible for ensuring that all personal and
sensitive personal information I submit, are accurate, complete and up-to-date.
Tôi hiểu và đồng ý rằng tôi là người chịu trách nhiệm đảm bảo mọi thông tin cá nhân và
thông tin cá nhân có tính nhạy cảm mà tôi cung cấp là chính xác, đầy đủ và cập nhật.

• I hereby agree and consent to and authorize Zuellig Pharma (“the Company”) to collect,
record, organize, store, update or modify, retrieve, consult, use, consolidate, block, erase
or destroy my personal and sensitive personal information provided by me or possessed
by the Company to perform its business processes effectively, safely and efficiently and
in accordance with Company policies.
Tôi bằng văn bản này đồng ý và thỏa thuận và cho phép Zuellig Pharma (“Công ty”) thu
thập, ghi chép, sắp xếp, lưu trữ, cập nhật hoặc sửa đổi, khôi phục, tư vấn, sử dụng, hợp
nhất, ngăn chặn, xóa bỏ hoặc hủy bỏ thông tin cá nhân và thông tin cá nhân có tính nhạy
cảm của tôi do tôi cung cấp hoặc do Công ty sở hữu để thực hiện công việc kinh doanh
một cách hiệu quả, an toàn và tuân thủ chính sách của Công ty.

• I acknowledge that my personal and sensitive personal data may or will be disclosed, as
may be necessary, by the Company to partner Companies, business partners, and
government agencies, as reasonable and necessary to perform its business processes
effectively, safely and efficiently and in accordance with Company policies.
Tôi chấp nhận rằng thông tin cá nhân và thông tin cá nhân có tính nhạy cảm của tôi có
thể hoặc sẽ được Công ty tiết lộ, khi cần thiết, cho các Công ty đối tác, đối tác kinh doanh
và cơ quan nhà nước, trong trường hợp chính đáng và cần thiết để thực hiện công việc
kinh doanh một cách hiệu quả, an toàn và tuân thủ chính sách của Công ty.

• I give my consent to the Company to use my photograph whether taken at company or

work-related events, or taken for purposes of producing Company-related identification.
Tôi cho phép Công ty sử dụng hình ảnh của tôi được chụp tại công ty hoặc tại các sự kiện
liên quan đến công việc hoặc được chụp với mục đích tạo ra nhận dạng liên quan đên
Công ty.

• I hereby also warrant that I am duly authorized to disclose the personal and sensitive
personal information of my dependents/beneficiaries and I hereby consent to the

 processing of such personal information in relation to any employee-related benefits
provided by the company. None of my dependents/beneficiaries shall dispute the
validity, enforceability or operations-related thereto nor contest the validity or legality
of the disclosure of their personal and sensitive personal information in connection
therewith.
Tôi bằng văn bản này cũng đảm bảo rằng tôi được ủy quyền hợp pháp để tiết lộ thông
tin cá nhân và thông tin cá nhân mang tính nhạy cảm của người phụ thuộc/người thụ
hưởng của tôi và tôi bằng văn bản này cho phép việc xử lý những thông tin cá nhân đó
liên quan đến các phúc lợi nhân viên do Công ty chi trả. Không có người phụ thuộc/người
thụ hưởng nào của tôi có tranh chấp về tính hợp lệ, hiệu lực hoặc các điều khoản liên
quan đến hoạt động cũng như không tranh cãi về tính hợp lệ hoặc hợp pháp của việc tiết
lộ thông tin cá nhân và thông tin cá nhân mang tính nhạy cảm của họ.

• I recognize that if I do not give consent to the collection, processing and storage of my
personal and sensitive information, it may affect my employment and administration of
statutory and company-provided benefits.
Tôi chấp nhận rằng nếu tôi không cho phép thu thập, xử lý và lưu trữ thông tin cá nhân
và thông tin cá nhân mang tính nhạy cảm của tôi thì công việc, việc thi hành pháp luật và
phúc lợi từ Công ty của tôi có thể bị ảnh hưởng.

• All of my own and my dependents’/beneficiaries’ personal and sensitive personal

information will be retained by the Company as long as necessary for the purpose of
maintaining records and to comply with applicable laws, rules and regulations.
Tất cả thông tin cá nhân và thông tin cá nhân mang tính nhạy cảm của tôi và của người
phụ thuộc/người thụ hưởng của tôi sẽ được Công ty lưu giữ trong khoảng thời gian cần
thiết cho mục đích lưu trữ và để tuân thủ với các quy định, điều lệ và pháp luật hiện hành.

• I also recognize that the Company will cease to retain my personal and sensitive personal
information, as soon as it is reasonable to assume that such retention no longer serves
the purposes for which the personal data were collected, and are no longer necessary
for legal or business purposes. After this time, my personal information shall be removed
from the electronic recording system of the Company and all the physical records thereof
shall be destroyed completely.
Tôi cũng chấp nhận rằng Công ty sẽ chấm dứt lưu giữ thông tin cá nhân và thông tin cá
nhân mang tính nhạy cảm của tôi ngay khi có lý do chính đáng để cho rằng việc lưu giữ
như vậy không còn phục vụ mục đích thu thập ban đầu và không còn cần thiết cho mục
đích pháp lý hoặc kinh doanh. Sau thời điểm này, thông tin cá nhân của tôi được gỡ bỏ
khỏi hệ thống lưu trữ điện tử của Công ty và mọi ghi chép trên mọi vật liệu của các thông
tin đó cũng bị hủy bỏ hoàn toàn.

• I hereby consent to the collection, processing and storage of my personal and sensitive
personal information by the Company and hold the Company free and harmless from
any liability that may arise therefrom. Otherwise, I shall indemnify the Company for any
complaint, suit or damages any party may file or claim in relation to my consent.

 Tôi bằng văn bản này cho phép Công ty thu thập, xử lý và lưu giữ thông tin cá nhân và
thông tin cá nhân mang tính nhạy cảm của tôi và không ràng buộc Công ty chịu bất kỳ
trách nhiệm pháp lí nào phát sinh từ việc thực hiện các công việc trên. Nếu không, tôi sẽ
bồi thường cho Công ty về mọi khiếu nại, kiện cáo hoặc thiệt hại từ một bên bất kỳ liên
quan đến thỏa thuận của tôi.

• By signing below, I hereby confirm that I understand the terms as stated above and that
I am voluntarily giving my consent to the collection, generation, use, processing, storage,
and retention of my personal and sensitive information by the Company. I also
understand that this consent is to allow the Company to process my current and future
benefits more efficiently, with less inconvenience on my part.
Bằng cách ký tên dưới đây, tôi bằng văn bản này xác nhận rằng tôi hiểu rõ những điều
khoản trên và tôi tự nguyện đồng ý, cho phép Công ty thu thập, tạo lập, sử dụng, xử lý,
lưu trữ và lưu giữ thông tin cá nhân và thông tin cá nhân có tính nhạy cảm của tôi. Tôi
cũng hiểu rằng thỏa thuận này cho phép Công ty xử lý quyền lợi hiện hữu và quyền lợi
trong tương lai của tôi một cách hiệu quả và thuận lợi hơn cho cá nhân tôi.
