This letter is to inform you of your Unemployment Insurance rights and responsibility regarding out of state
registration for work. In order to satisfy the qualifying provisions of the law, you must meet the work registration
requirements of the state where you live.

To remain eligible for unemployment insurance benefits, you must register online or visit the nearest One-
Stop Center or State Workforce Office in the state where you currently reside to register for work. To find
the office nearest you, visit the Department of Labor web site at http://www.servicelocator.org/OWSLinks.asp.
This web site displays a map and state/territory links to locate the state office nearest you. If you do not have
internet access, you may contact the Nevada Unemployment Insurance Operations Center at (775) 684-0350
for assistance.

Please be advised, failure to register for work may result in a delay or denial of benefits. If you have not
already done so, you must register within ten (10) days of the mailing date of this letter.

Please provide either a screen shot or a paper copy of your registration with the Job Service agency in your
state.
