 Provide company’s letterhead at the start of the document

 Date the document on the top left side of the page

 Provide the name and address of the recipient below date

 Follow letter format, start with a greeting

 Provide personal information about patient

 Provide medical history of patient 

 Professionally toned with formal language

 Use a heavy amount of medical terms

 Keep paragraphs small and short

 Align all paragraphs at the left side of page

 No indentations 

 Single spaced

 No graphics in document besides the company logo

 Provide a detailed summary of treatment plan of patient

 Table of medical history with diagnosis, duration and

medication
 End document with salutations and e-signature
