depose and say:

1. That I am the authorized person to represent our company (LARES) and execute this undertaking;

2. That as part of our contract as the official contractor of the computerization project of the Land
Registration Authority, it is our duties and responsibilities to set up the computers, LAN connection and
Server of each and every Registry of Deeds in the Philippines;

3. That in relation to the above stated duties and responsibilities, our authorized IT personnel will be
setting up the computers, LAN connections, and Server in the Registry of Deeds Manila, located at the 2 nd
Floor of the City Hall of Manila;

4. That I DECLARE and ASSUME full and complete responsibilities for all consequences which
may arise in connection with the above-mentioned work or activities or any damage that our personnel may
cause to any property of the said office;

5. That I execute this Affidavit of Undertaking in compliance with the requirements in the issuance
of an authority to conduct the setting up of computers, LAN connections, and Server in the Registry of
Deeds Manila.
