AFFIDAVIT OF LOSS

I, ANNABELLE S. ALON, Filipino, of legal age, and with residence

address at 1023 Sto. Niño St., Muntinlupa City, after having been sworn to in
accordance with law, hereby depose and state:

1. I am the owner of a papel de ahenya issued by ETG

Pawnshop, Inc. Branch 2 with PT# 14701 covering a set of ring
and earring pawned on 28 December 2019 for Six Thousand Five
Hundred Pesos (Php/6,500.00).

2. I would usually keep said papel de ahenya inside my

purse.

3. Unfortunately, upon searching for my papel de

ahenya, I could no longer find the same despite exertion of utmost
effort and due diligence.

4. I hereby attest that said papel de ahenya is already

lost and could not be retrieved.

5. I am executing this affidavit to attest to the truth of

the foregoing facts and for whatever purposes it may serve.
