1. In connection with the request conduct verification of firearm used in the

commission of crime described below;

a. DESCRIPTION OF FIREARM FOR VERIFICATION: Pistol Caliber

9mm Glock 17 Gen 4 with SN PNP59566

b. NATURE OF CASE: Illegal Possession of Firearm (RA 10591)

c. SUSPECT: Ms. Heart M. Evangel, 35 years old, married,

mambobonong, resident of OD-432, Purok 4, Barangay Talipapa,
Baguio City

d. TDPO: At about 7:00 PM of March 28, 2020 at JP Soriano St.

2. This is to certifies that Ms. Heart M. Evangel DOES NOT have any License
To Own and Possess Firearm (LTOPF) or any Permit to Carry Outside
Residence (PTCOR) registered or any pending application for such license
and permit under this Office of the Philippine National Police.

3. This is to certifies that the firearm (9mm Glock 17 Gen 4 with SN PNP59566)
found under the possession of Ms. Heart M. Evangel DOES NOT have any
record, registration or pending registration under this Office of the Philippine
National Police.
