AFFIDAVIT

I, Dr. Suvarna Suryakant Kakde, Age 35 years and Residing at : Aakash Chs,
Flat no 103, Plot No 8 and 9, Sector – 42, Seawoods west, do hereby state and
declare on solemn affirmation as under:
That I have passed M.D Ayurved Agadtantra avum Vidhivaidyaka
(MUHS Nashik ) from Tilak Ayurved college, Pune in the academic year
October 2012.

I further say that from October 2012 to october 2020 is my gap period, during
my gap period I did not join any educational institute in Maharashtra or
elsewhere due to personal reason.

Hence, I am making this affidavit to produce the same before the concerned
authorities To Whom SO ever It May Concern to consider my gap period from
October 2012 to October 2020 so as to enable me to pursue of this affirmation
for further my Ph.D. studies at- D.Y.PATIL Deemed to be University, Nerul,
Navi Mumbai.

Whatever stated here in above is true and correct to best of my knowledge and
belief.
