R/o Amar Bharti, Prabhu Colony, P.O. - Mohabbey Wala, Dehradun under
the following declaration:-

1. I have been briefed about the course contents and duration of


community development programme through polytechnic sponsored
by ministry of skill development got and organized at scholars tech,
Dehradun in collaboration with Pithuwala polytechnic, Dehradun.
2. I commit to be regular for the program and continue until completion
of the course.
3. I have been informed and I declared that I am not required to pay
any kind of feels for admission and neither will I be asked to do so by
the institute during the entire duration of the program i.e. to months.
4. I have been informed that if I am absent for 07 days continuously, my
registration my registration from the said course would be cancelled.
5. I am aware that in the event of withdrawing from the course before
the completion of the prescribed during I might be fined for Rs.
1000/-
