DENVER II (1998) PRESCREENING DEVELOPMENTAL QUESTIONNAIRE (PDQ-II) INFORMATION FOR PROFESSIONALS The revised Denver Prescreening Developmental Questionnaire (R-POQ) has boon newly revised to update monitoring and screening of children's development. The PDG-Ilis designed to achieve three goais: 1) to make parents more aware Of the development of their children; 2) to document the developmental progress of individual children in a systematic manner; and 3) to facilitate earlier identification of children whose development may be delayed. INSTRUCTIONS FOR PREPARATION OF THE PDQ-II 1 Use the following guide to determine which of the four PDQ-II forms is appropriate for the child: Orange (0-9 MONTHS); Purple (9-24 MONTHS); Cream (2-4 YEARS); White (4-6 YEARS) 2, Select the appropriate form and write the child's name, today’s date, and the child's birthdate at the top of the PDQ-II form. 3. Complete the age calculation in the box on the front of the form, Figure the number of days first, then the number of months, and finally the number of years. For PDQ. resuits to be quickly and correctly interpreted (ses back of form), itis necessary to convert the child's exact age to a PDO-II age. For children below 24 months of age, the PDO-II age is expressed in completed months and weeks, as shown: Child's Exact Age: lyr 8mo 20day PDO-II Age: -yr 20mo 2 completed weeks For children above 24 months of age, the PDQ-II age is expressed in years and months. Thera is no need to write in the completed weeks after 24 months of age. Child’s Exact Age: 3yr 6 mo. 28 day PDQ-II Age: 3yr 6 mo. - completed weeks DO NOT ROUND OFF TO THE NEXT HIGHER AGE TO EXPRESS THE PDQ-II AGE. ONLY COMPLETED WEEKS, COMPLETED MONTHS, AND COMPLETED YEARS SHOULD BE CONSIDERED, 5. Give the prepared form to the child's caregiver and ask that (s)he note who is completing the form, and his/her relation to the child. The caregiver should then answer questions until: 1) 3 "NOs” are circled (they do not have to be consecutive); or 2) all of the questions on both sides of the form have been answered. 6. Check form to see that all appropriate questions have been answered. INSTRUCTIONS FOR INTERPRETING PDQ-Il RESULTS. The items on the PDG-II are arranged in chronological order according to the ages at which 90% of the children in the DENVER i" standardization sample could pectorm them. A "NO" response may signify a delay. Steps in interpretation are as follows: 1 Review “YES” and "NO" responses. Assure that the child's caregiver understood each question and scored the items correctly. Give particular attention to the scoring of questions that require verbal responses by the child, and that require the child to draw. 2. Identify “delays.” Ages at which 90% of children in the DENVER II sample passed the items are listed under "90%" in the “For Office Use” column. These ages are shown in months and weeks up to 24 months (for examale, 20-2" indicates 20 months, 2 weeks), and in years and months after 24 months (for example, “3y-6" indicates 3 years, 6 months). A “delay” is any itam passed by 90% of children at a younger age (by even one day) than the child being screened. Highlight “delays” by circling the 90% age to the right of the item that the child was not able to perform. 3. Optional - Identify “cautions.” Ages at which 75% of children in the DENVER Il sample passed the items are listed under “7596” in the “For Office Use" column. A “caution” is any item passed by 75% of children at a younger ‘age (by even one day) than the child being screened. A few age-appropriate developmental activities may be suggested to their caregivers. Ita child has one “delay” or two “cautions,” the caregiver should be given age-appropriate developmental activi- ties to pursue with the child, and the child should be scheduled for resoreening with the PDQ-II one month later. IM, on rescreening a month later, the child has one or more “delays,” schedule second-stage screening with the DENVER Il, administered by a qualified examiner, as soon as possible. 6 Children who have two or more “delays” or three or more “cautions” on the first-stage soreening with the PDG-ll should be scheduled for a second-stage screening with the Denver Il as soon as possible 7. on second-stage screening with the DENVER Il, a child receives other than normal results (suspect or untestable), that child should be considered for a diagnostic evaluation.
