MOTORCYCLE WITH SIDECAR and to attest to the truth of the foregoing and
in support of my request to change the entry in the body type portion/section to
MOTORCYCLE WITH SIDECAR in the motorcycle’s OR/CR; and,

4. AFFIANT SAYETH NAUGHT.
