Details of patient - Mr. Daniel Steele; 63 years old man, independent, recently retired, worked as a joiner,
married

Source of the history- patient

Chief complaint- Haemoptysis and suspected bronchiectasis on chest X-ray

Present illness-

1) Coughing up blood for the past 4 weeks.


2) Blood first time, small amounts, several times a day
3) Persistent cough around 2 years
4) First time blood in cough
5) Sometimes sputum turns green
6) Feeling about clearing chest each morning,coughs a lot
7) Feels like breathless on exertion for around 1 year.
8) On breathlessness feel wheezy
9) Feels no pain, no ankle swelling, no weight loss
10) 6-7 courses of antibiotics over past 2 years.
11) Smoke- 15 cigarettes/day- 17 yrs. Age
12) Alcohol ara
Past History-
13) CHILDHOOD ILLNESS – 1 episode of severe pneumonia
14) Adult illness – continuous cough for around 2 years
15) Surgical- nothing
16) Obstetric/Gynaecologic- nothing
17) Psychiatric- nothing
18) Immunisation- nil
19) Screening test- nil
Family History- none
Personal & Social History-
20) Retired, worked as joiner
21) Lives with wife, independent, has dog from past 8 years
22) Smoke 15 cigarettes per day
23) Travelling- was were in Paris 6 months ago
