GRANT OF LICENCE TO PRIVATE SECURITY AGENCY TO OPERATE IN UTTAR

PRADESH

Proprietor/ Partner/Director of M/s do hereby solemnly affirm

and declare as under :-

1. That the deponent is a citizen of India.

2. That the deponent has not been convicted of any offence in connection with the
promotion, formation of Management of a company (any fraud or misfeasance committed
by him in relation to the company) including an undischarged insolvent.
3. That the deponent has not been convicted by a competent court for an offence, the
prescribed punishment for which is imprisonment of not less than two years.
4. That the deponent has not been keeping links with any organization or association which
is banned under any law on account of their activities which pose threat to national
security or public order of there is information about such a person indulging in activities
which are prejudicial to national security or public order.
5. That the deponent has never been dismissed or removed from Government service on the
grounds of misconduct or moral turpitude.
6. That I ensure that the facility of the imparting of training and skills of a private security
guards and its supervisors required under Rule 5 of the Punjab Private Agencies Rules,
2007 and Private Securities Agencies (Regulation) Act, 2005 are available/ will be made
available within the specified time period as given in the Private Security Agencies
Regulation Act, 2005 and Punjab Private Security Agencies Rules, 2007
7. That the deponent shall comply with the provisions of the Private Security Agencies
Regulation Act, 2005 and the rules (Punjab Private Security Agencies Rules, 2007)
framed there under by the State Government and instructions issued from time to time by
the Controlling Authority, Punjab appointed under the Act, while managing private
security agency with the name and title M/s .
