MED / SUR Case Presentation Marking Criteria

Students are required to write up two case reports during CP4 (one medical case and one surgical
case). They must send their soft copy of case report to their Liaison before the fifth week in
Med/SUR ward. Then they have to submit the hard copy and present the case report to Liaison
before the sixth week in MED/SUR ward. Besides, the care plan must submit 5 working days after
the presentation.

Student will be assessed based on the following included items:

1. Admission Information must include:

▪ Type, date and reason for admission
▪ Admission diagnosis
▪ Baseline vital signs

2. Background Information must include:

▪ Medical history, allergy and infection precaution
▪ Social history (marital status, accommodation, financial support, emotional status,
smoking/alcohol/substance abuse)
▪ Medication history

3. Assessment, planning & intervention must include:

▪ Skin integrity/nutrition/risk of fall/pain
▪ Relevant investigation and laboratory results
▪ Refined diagnosis
▪ Relevant interventions provided and referral

4. Able to formulate and prioritize nursing Diagnosis (at lease 5) in PES format.

5. Able to provide current condition and discharge plan
