Nursing Diagnosis:

Impaired urinary elimination related to infection in the urinary tract as manifested by dsyuria,
increase urgency, increase frequency and verbalization of “Mamin ado ak agsubli-subli ta CR ken nasakit
nu umisboak”.

Nursing Inference:

The invasion of microorganism in the urinary tract could irritate the tissue initiating the
inflammatory response. This results to increased irritability of the bladder and other structures
of the urinary system resulting to the impairment of urine elimination.

Nursing Goal:

After 3-5 days of rendering nursing intervention, the patient will be able to have
improved urinary elimination pattern as will be manifested by absence of pain during urination,
decrease urgency and frequency and verbalization of ”Mimmayat ti panagisbo kon ken haan a
nasakit nu umisboakun.”

1. Administer anti-infectives such as To inhibit the growth and multiplication of the
Ciprofloxacin as prescribed. invading microorganism by its killing action.
2. Instruct to wipe front to back after To promote proper perineal hygiene thus
urination or bowel elimination. avoiding the risk for re infection.
3. Encourage increase vitamin C intake. To maintain the acidity of the urine.
4. Encourage to increase oral fluid intake. To lessen the risk of re infection.

Nursing Evaluation:

After 5 days of rendering nursing intervention, the patient was able to have improved
urinary elimination pattern as manifested by absence of pain during urination, decrease
urgency and frequency and verbalization of “Mimmayat ti panagisbo kon ken haan a nasakit nu
umisboakun.”
