for my son / daughter to attend the classes through physical mode in the campus of
Oxford Public School, ............................

We shall be responsible for the health and well-being of our son/daughter

and are also aware of the COVID protocols laid by the Government, etc., We shall
co-operate with the school in all aspects.

As a parent, I am aware that the number of Corona cases are increasing in

the State Everyday. I understand that it is a huge risk to send my child physically to
school. The School has also provided option to attend the Classes ONLINE. As a parent
I have refused that option and i am willing to take full responsibility for my child. The school
has also recommended against sending my child to school , as there is NO VACCINE and
NO CURE for the Corona Virus. Sending my child physically to school is 100 % my
decision and will not hold the School or any other responsible if my child is infected or any

of my family members are infected due to this.
