Student Name: Goals:

Deen Chadwick By February of 2021, In the classroom or at CBI, When given 3 real-world math
(Galdames) scenarios involving money, then asked a 1-2 step addition, subtraction,
multiplication and division question regarding the scenario, Deen will solve for
each problem at 80% accuracy across 2 weeks given access to a calculator or an
app. (i. 2 for $5 and the other deal is 3 for $10, which is the better deal? or the
video game is $20, it is 10% off, how much will you pay? 1lb costs .99c, how
much does 2lbs cost?)

Laura Mania By 10/15/2020, when given a word problem involving addition or subtraction of
(McDonald) two fractions with like denominators, Laura will solve the word problem using a
visual model (e.g.,fraction bars, number line, or student drawn images) with 75%
accuracy as measured by teacher work samples.

Roberto Montoya By 4/26/2021, given visual supports, teacher support and a calculator, Roberto
(Kraushaar) will solve real-world and mathematical problems involving the four operations
(+,-,x, and /) with rational numbers with at least 75% accuracy as measured by
student work samples/assessments.

Edward Raigosa By 12/19/20, when given fractions 1/2, 1/4 and 3/4, Eddiy will shade in a diagram
(Kraushaar) or picture and be able to apply the fraction to a real life application with 60%
accuracy in 3 out of 4 trials as measured by student work samples and teacher
observation.

Jaden Savill By 10/30/2020, after correctly solving a two-step word problem involving whole
(Valerie) numbers and/or decimals (e.g. "Tim goes to the store and purchases 5 baseballs for
$1.27 each. If he pays for his purchase with a twenty dollar bill, how much change
will he get back?"), Jaden will use manipulative money to count and determine the
correct amount of money for the change, using the least amount of bills and coins
for (4 out of 5) word problems.

Vincent Thomas By 10/17/2020, when given (5) problems using positive and negative numbers to
(Saul) represent quantities in real-world contexts (e.g. temperature, elevation, money),
Vince will correctly add and subtract positive and negative numbers for (4 out of
5) problems.

Kayson Truelove By 01/30/2021, when given (5) two-step word problems involving whole numbers
(McDonald) and whole number answers that require the use of the four operations (e.g.÷, x, +,
-), Kayson will show written work and the correct final answer with 75% accuracy
as measured by teacher data.
Bryce Walker By 11/3/2020, when given 10 multi-step, whole number word problems involving
(Galdames) the four operations, Bryce will solve each problem correctly for 4 out of 5
problems presented, over a two-week period, as measured by work samples.
