BIOGRAPHIC DATA
• PATIENT F.A.B , IS 80 YEARS OLD MALE , WHO WAS BORN ON SEPTEMBER 18 , 1939 IN
CEBU CITY. HE IS MARRIED AND CURRENTLY RESIDING AT GUANDALAJARA , CEBU CITY . HE IS A
FILIPINO AT BIRTH AND ROMAN CATHOLIC BY RELIGION. BOTH PARENTS ARE DECEASED. THE
PATIENT CAME TO ER ACCOMPANIED BY HIS WIFE ON AUGUST 11, 2020 AT 3:53 PM , WITH A
CHIEF COMPLAINT OF COUGH AND DYSPNEA. PATIENT’S BASELINE VITAL SIGNS ARE AS
FOLLOWS: BP : 130/90MMHG , HR : 121BPM , RR: 36 RPM, TEMP: 36.7 DEGREE CELCIUS , O2
SAT : 75%. THE PATIENT WAS ASSESSED BY THE PHYSICIAN DR FERNANDEZ AT THE IM

PATIENT’S OTHER RESULT WAS IN NORMAL RANGE.
IMMUNOLOGY

• PATIENT’S IMMUNOLOGY EXAMINATION HAS A RESULT OF QCRP >160 MG/DL (NORMAL VALUE
<10), THE INCREASE OF IT INDICATES THAT THERE IS INFLAMMATION IN THE PATIENT’S BODY
CAUSED BY SOME INFECTION. PATIENT ALSO UNDERGONE COVID-19 RAPID ANTIBODY TEST
(QUALITATIVE) AND THE RESULT WAS POSITIVE IN THE EXAMINATION.

• PATIENT’S FAST BLOOD SUGAR RESULT WAS 142.00 MG/DL (REF. RANGE: 70-100).HIS
TRIGLYCERIDES WAS 221.8 MG/DL (REF. RANGE: 25-148).
BLOOD GAS ANALYSIS
• PATIENT’S BLOOD GAS ANALYSIS HAS RESULT OF A DECREASED PCO2 LEVEL OF 18.5MMHG
(NORMAL VALUE 35-45), PO2 WAS 115MMHG (NORMAL VALUE 80.0-100.0), HCO3 WAS 10.8
MMOL/L (NORMAL VALUE 22.0-26.0).
LDH OR LACTATE DEHYDROGENASE
• PATIENT’S EXAMINATION FOR LDH OR LACTATE DEHYDROGENASE HAS DRASTICALLY INCREASED
WITH THE RESULT OF 1560. 09 (NORMAL VALUE <480), THIS INDICATES THAT THERE IS TISSUE
DAMAGE ON THE PATIENT. THE PATIENT WAS ALSO POSITIVE FOR TROPONIN-1 WITH A RESULT
OF 521.30 NG/L. PATIENT'S SODIUM LEVEL WAS 137 MMOL/L, POTASSIUM LEVEL WAS 3.30
MMOL/L, CREATINE WAS INCREASED 2.00 (REF. RANGE: 0.51-0.95), BUN WAS INCREASED 27.6
(REF. RANGE: 6.0-20.0), BUA WAS ALSO INCREASED WITH A RESULT OF 8.70 (REF.RANGE: 2.4-5.7)
AND LASTLY THE ALT (SGPT) OF THE PATIENT WAS ALSO INCREASED WITH A RESULT OF 26.60

NORMALIZED RATIO (INR) OF 1.37 SECONDS. THE PERCENTAGE ACTIVITY HAS DECREASED INTO
72.99% THAN THE NORMAL RANGE. PATIENT'S PLATELET RESULT WAS 110X 10/MM3 AND IS
MUCH DECREASED THAN THE NORMAL RANGE.

physicians and agencies. A written discharge will
be provided.
