Social worker : Miss Rothschild. Hi nice to meet you. I’m Karen yin, and I’m the social worker
here. Before you go in to see the doctor we have a few questions. That we
started asking patients? I have them right here in front of you. So, these are
questions, we want to ask to find out more about who our patients, are what
needs do they have how can we provide quality care. Does that sound okay to
you?

Patient : Sure, that my appointments in 10 minutes, is there time?

Social worker : Oh yes, it’ll only take a few minutes I just want to point out that for any of the
questions you can see prefer not to answer and I do want to let you know that
the only people who can see this in your file are the people who take care of
you while you’re in our care.

Patient : Okay

Social worker : Otherwise it’s used for research and nothing can be linked back to you
personally. So, the first question is what language would you feel most
comfortable speaking in with your health care provider?

Patient : English

Social worker : Okay, which of the following best describes your racial or ethnic group?

Patient : A white European.

Social worker : It’s your plan okay. Thank you, were you born in Canada?

Patient : No.

Social worker : Okay, so what’s your did you arrive here?

Patient : At 72, 73.

Social worker : Okay, an approximate this once output 73. so what does your religious or
spiritual affiliation?

Patient : I’m Jewish.

Social worker : Jewish, okay so on the next page you can flip that do you have any of the
following disabilities?

Patient : I have diabetes.

Social worker : You want to put that down as a chronic illness maybe I usually?

Patient : Okay.
Social worker : Okay, what does your gender?

Patient : Female.

Social worker : Okay and what’s your sexual orientation?

Patient : I’m normal.

Social worker : All of them are normal diet so what we really want to know is why you
attracted. So did you mean to say may be heterosexual. So female, male?

Patient : Yes

Social worker : Yeah, okay thank you what’s your total family income before taxes last year.

Patient : And number two thirty thousand to 60.000.

Social worker : Okay um how many people does this income support.

Patient : 2.

Social worker : two? Ok just a few more questions and I’ll answer and what would you prefer
to read health care information.

Patient : English.

Social worker : English, and what type of housing do you live in?

Patient : I…Eumm.

Social worker : So again we have a list it can be owned and anything.

Patient : I we all know.

Social worker : Okay and that’s it so you can just go and have a seat until someone calls your
name and thank you very much.

Patient : Okay thank you.

Social worker : Thank you.
