Hours of the experience: 12 Objectives: CCPO #2 For most of the rotation, I was placed with our Nursing Informatics leader to review and learn to create a request form for a particular evaluation form to be created and entered into the EMR. Our Nursing Informatics leader reviewed the process including identification, evaluation, priority, and creation of a patient form that is being suggested for EMR entry. The process has a lot to do with patient safety and quality. Each form, which is presented to nursing informatics, is broken into particular points and sections, given a form #, and a bar code created. After identifying the form as a necessary permanent record of a patient's chart, you then identify a priority. The form we worked on is identified as priority 1. The form created is an ISAR form, which is currently a paper form. The priority was chosen due to the patient safety, inner team communication needed, and specific certification for the Geriatric ED needed. Next, we added a physical description of the form along with the information necessary to capture. The form was requested to have a scan barcode and was to populate automatically to be completed for all patients age 65 years of age and older. This process became frustrating at times due to the fact certain spaces only accepted a certain number of characters. After completing the IT and EMR forms for the ISAR we then scanned the paper form into the computer and attached it to the request sent to CERNER to create and send back. Technology now with EMR is challenging to understand at times. The nursing informatics leader reviewed and helped me identify the process of creating forms. She also helped clarify and bring a deeper understanding of the difference between nursing informatics and IT. . Objectives: CCPO # 5 The remainder of the shift the house supervisor and I talked about continuing my education for my Master's degree and certain criteria that is needed for the specialty I am interested in which is education. The nurse supervisor also advised me to research other possible Master's degree avenues and schools. She is very knowledgeable on career pathways and potential schools.
