I, the undersigned, hereby declare that I agree to participate in the evaluation of the above-mentioned
tender procedure. By making this declaration, I confirm that I have familiarized myself with the
information available to date concerning this tender procedure. I further declare that I shall execute
my responsibilities honestly and fairly and in accordance with the Programme and Operations Policies
and Procedures sections regarding the UN Staff Regulations and Rules, the UNDP Financial
Regulations and Rules, the rules and Manual for the type of contract being awarded, and the UNDP
Fraud and Corruption Prevention and Policy, all of which I am aware of.

I am independent of all parties, which stand to gain from the outcome of the evaluation process. To
the best of my knowledge and belief, there are no facts or circumstances, past or present, or that could
arise in the foreseeable future, which might call into question my independence in the eyes of any
party; and, should it become apparent during the course of the evaluation process that any past,
present or future relationship has been established, exists, or that could arise in the foreseeable future
between myself, my family members, organization or company that I am involved with outside of my
position with UNDP and any of the proposing organizations or their subcontractors or affiliates, I will
immediately declare this and notify the Chairperson of the Evaluation Committee, and cease to
participate in the evaluation if requested. The Chairperson of the Evaluation Committee will consult
with Senior Management and I will abide by the decisions of Senior Management. I confirm that I,
my family members and no organization or company that I am involved with outside of my position
with UNDP shall make any financial gain from the results of this tender process.

I agree to hold in trust and confidence any information or documents (“confidential information”)
disclosed to me or discovered by me or prepared by me in the course of or as a result of the evaluation
and agree that it shall be only used for the purposes of this evaluation and shall not be disclosed to any
party, except with the knowledge and consent of the Chair and Senior Management. I also agree not to
retain copies of any written information or prototypes supplied.

Confidential information shall not be disclosed to any employee or expert unless they agree to execute
and be bound by the terms of this Declaration.

I am aware that the non-compliance with the above can be sanctioned as provided in the Programme
and Operations Policies and Procedures sections regarding UN Staff Regulations and Rules, UNDP
Financial Regulations and Rules, and UNDP Fraud and Corruption Prevention Policy, Prescriptive
content on accountability, disciplinary measures and procedures.
