I am a keen student at city of Bristol college studying FDA Professional photography,

currently I am working on a project called “A Mother’s Love” where I have been researching
the affects on a woman’s identity when experiencing Motherhood for the first time.
I am writing to ask if you would kindly take some time to assist me in my work by
completing the enclosed questionnaire. The data I receive from you will be kept anonymous
within my work and I will address each participant as subject a, b, c etc to ensure your
personal privacy. Once the project is completed I will destroy questionnaires and any
identifying information. If you wish you may withdraw at any time and request that your
data is not to be used. If you are able to participate please may you read and sign the
consent form attached and send it back to me along with the completed questionnaire.
