AFFIDAVIT OF LOSS

I, RAQUEL M. MERCADO, Filipino, of legal age, married, and with office address at
San Miguel City, after having been duly sworn to in accordance with law, do hereby deposes and
states:

1.) That I am the Accountant of Tarlac Distillery Corporation (“TADISCO”), a

corporation duly organized and existing under and by virtue of the laws of the
Republic of the Philippines;

2.) That being registered with the Bureau of Internal Revenue, TADISCO has been
issued a BIR Certificate of Registration last January 4, 1996, with RDO Control
Number 96-047-004865. A copy of which is attached herein as Annex “A”;

3.) That sometime in 2017, the original copy of the said BIR Certificate of Registration
could not be found and that despite of diligent search and efforts, the same could no
longer be found such that I now believe that it is now lost beyond recovery;

4.) As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing
and for whatever legal purposes that this may serve.
