The Medicare Payment Advisory Commission (MedPAC) reported in 2005 that about 11 percent of
hospital stays resulted in a readmission within 15 days of discharge and 18 percent within 30 days of
the patient leaving the facility, accounting for $17 billion in spending. In the hospital setting,
determining the readmission risk for a particular patient at the point of registration and after
discharge, would allow providers to monitor that patient di erently and could lead to better health
outcomes.

Medical and pharmacy big data paired with intelligent analytics has the potential to revolutionize
patient care. In this presentation we will discuss and demonstrate techniques used to improve
readmissions results, speci cally detailing the process used in aggregating the disparate data and the
role of analytics in answering business-critical questions:

• Impact of readmission on patients, providers and payers

• Data gathering issues (legal share of data/ privacy matters)
• Ingesting data from classical RDBMS to Hive; data validation and data cleaning; “fuzzy joining” data
from di erent systems
• Feature generation from raw data; feature enrichment using publicly available data sets
• Feature selection for predicting readmission risk
• Modeling readmission risk using predictive analytics on selected features
• Description of tools created to utilize the model; walk through some interesting results
