What is 5 Why Analysis?


5 Why Analysis is a traditional method in finding the root cause. It is essential to go to the actual
situation by using the 3 Actual ’s or 3 GENs.
In 5 Why Analysis always remember the key points.
- Checking the actual situation
- Consider all the main causes from a different view
- Declare Who?Which?What products and What materials, etc.
Without all of this, we have the risk of mistaking in finding the root cause of the problem.
In this picture, we Go to the actual site, We identify the actual material, Analyzing the actual situation
and come up with the root cause.

For the problem Tangled wires problem at Civic Floor L2, the root cause is hard to follow work
procedure, The corrective action formulate is to revision of I.S.

For the miscavity problem of Mitsubishi NS LHD, We identify the root cause which is defective CPG
jig. The corrective action formulated is to replace the CPG jig.
