ADAM’S FORWARD BEND TEST

Definition

An Adam’s Forward Bend Test is used to detect scoliosis. The patient takes off
his/her t-shirt so that the spine is visible. The patient needs to bend forward, starting at the
waist until the back comes in the horizontal plane, with the feet together, arms hanging and
the knees in extension. The palms are hold together. The examiner stands at the back of the
patient and looks along the horizontal plane of the spine, searching for abnormalities of the
spinal curve, like increased or decreased lordosis/kyphosis, and an asymmetry of the trunk.

Purpose

The purpose of the Adam’s forward bend test is note the functional scoliosis. It is
usually used in patients who have a family history of scoliotic posture or with patients who
have detectable scoliosis. The scoliosis is functional when the characteristics of scoliosis
become more visible while the patient bends. With a structural scoliosis, the scoliotic
deformity will remain the same as in the standing position.

Objectives

1. To detect patients who may have scoliosis.


Equipment
1. Scoliometer

PROCEDURE
 Explain procedure to the client.
Rationale: Explanation facilitates cooperation and provides reassurance
for patient.
 Introduce yourself to the client.
 Prepare the equipments.
 Provide client privacy.
 Ask the patient to remove their top to see their back clearly.
 Inspect the back. Note any asymmetry or unevenness in the shoulders,
ribs, neck, wasit and hips.
 Ask the child to bend forward to the waist as if they were trying to touch
their toes. If one side of the ribcage is noticeably higher than the other, it
is possible that it may be result to scoliosis.
 Ask the patient or assist the patient to put their clothes back.
 Document the result.
Nursing Responsibilities with Rationale
1. Assess respiratory status every 4 hours because scoliosis may hinder the
movement of the ribs.
2. Auscultate breath sounds to detect decreased or adventitious breath sounds.
3. Help the the patient in a semi-fowler postion to facilitate expansion of the lungs.
4. Administer oxygen if needed.
