 That I am a citizen of India.

 That I am known by the name Dharm Chand Gupta and have used the same in my
personal and official records.
 That my name in demat account is mentioned as Dharm Chand Gupta and while the
same is mentioned as Dharam Chand Gupta in the share certificates number 244821
& 244822 that I hold of the company Chambal fertilisers and Chemical Ltd.
 I hereby declare that both the names Dharm Chand Gupta and Dharam Chand Gupta
pertains to one and same person, the deponent.
