DAILY SAFETY TOPIC: Recordable Injury Incident Alert

RECORDABLE INJURY INCIDENT ALERT - 4700 FACILITY – CALGARY, AB


On Monday, May 6, 2018 at 11:45 hours, an employee was fitting a two-inch (2”) pipe.
The employee needed to grind a tack-weld and when he applied the grinding disk to the
weld, the clutch-guard of the grinder loosened and rotated, exposing the zip-disk edge.
The unexpected movement of the guard caused the employee ’s hand to slip resulting in
two (2) lacerations to the employee ’s left index finger. The employee sought out a First-
Aider and had his finger cleaned, treated and bandaged but it was determined the depth
of the cuts required sutures. The employee was transported to an Occupational Injury
Services (OIS) clinic for treatment by a physician (four sutures). The employee was
wearing cloth, cut-resistant gloves and full PPE at the time of the incident. This incident is
classified as MEDICAL AID – RECORDABLE.
Root Cause: Equipment & Maintenance: The grinder clutch-guard set-screw had been
replaced with a course-thread screw, whereas, the original was a fine-thread screw.
This thread incompatibility contributed to the loosening of the guard.
Corrective Action: Pre-use, employees are to check the grinder guards to ensure the
set screw has the proper thread and the clutch guard is secure. If the set screw needs
replacing, or any other deficiency is noted, tag it out of service and complete a
maintenance request form to action the repair.
