Consent Form

Any Brandon University applicant may designate a proxy (or two if necessary) to act on their behalf
during the admissions process. Applicants 18 years of age or older must complete the Admissions
Consent Form before information can be released to a third party.

This form must be completed and signed by the applicant, and returned to the Admissions Office at
admissions@brandonu.ca

This authorization will remain valid indefinitely. Should you wish to terminate this authorization,
you must make such a request by signed letter submitted to the Admissions Office.
