GOWN RENTAL AGREEMENT

1. Your complete bridal gown rental includes:

a. gown
b. alterations
c. steaming and pressing services

2. Accessories (i.e. veil, tiara, etc.), bra cups, crinoline slips, and/or shoes can be ordered separately and are for
purchase only.

3. The cost of a complete bridal gown rental is $425.00 plus tax.

4. Rental reservations placed within one (1) week prior to the event date is considered a rush order and will incur
an additional fee of $75.00 plus tax.

5. Once the rental reservation is made, the credit card submitted with the rental gown order form will be
automatically charged for the full rental cost plus a $200.00 security deposit.

6. The security deposit will be returned in the same form it was received once the gown is returned in acceptable
condition.

7. The first fitting should be scheduled at least two (2) weeks in advance of the event date, with the exception of
rush orders. The final fitting should be scheduled at least one (1) week prior to the event.

8. Rental return is expected the day after use. Saturday and Sunday wear dates must be returned on Monday.
There will be a $25.00 charge per day for late returns.

9. Renter agrees that the gown rented will be used in a manner for which it was reserved for, and will return the
gown in the original garment bag it was received. Renter is responsible for any repairs or replacement costs if
gown is damaged or lost.

10. Renter is responsible for the return of the rented gown to The Bridal Boutique at 614 Cooke Street, Suite 203,
Honolulu, Hawaii 96813.

11. Rental cancellations must be made no less than one (1) month prior to the event date. If cancellation is made
less than a month inclusive with the wear date, the renter will be charged the full rental gown cost.
