Description

The level of glucose in blood is monitored by the use of glucometer. It only requires a small sample and
by using that sample it can easily determine the level of glucose concentration in the blood.

Indication:

It is usually used with the clients that come with the diabetic problems. It is also used for the clients that
are having the episodes of hypoglycemia and hyperglycemia.

Considerations:

Nursing Interventions (pre, intra, post)

Pre: in this you have to check the orders first, then you need to gather some supplies and then inform
the patient about the procedure. Another important step is to perform the proper hand hygiene.

Intra: carefully select the site or the target area first. After that, clean the area properly. Always try to
use a new device of lancet. Now carefully wipe off the drops of blood. Collect the sample that is required
and after you are done with the sample, apply the gentler pressure on the targeted area so that extra
blood does not waste away.

Post: make sure that there is no further loss of blood, keep a check on the order of medication, and
always take care of hand hygiene.

Outcomes/Evaluation:

The level of glucose in blood should be in the range of 70-100.

Client education:

Properly learn the use of glucometer so that it can be used at home. Always use new lancet devices.
Take care of the hand hygiene. Always keep the record of the level of blood glucose. Dispose off the
lancet devices every time properly. Always report on time to HCP about the problem.

Potential complications:

Sometimes a punctured site can get an infection. The reading that were recorded could be inaccurate.

Nursing Interventions:

There should be a proper education on the hand hygiene. Before collecting the sample always use the
alcohol wipes. Make sure that the strips you are using are not expired. Strips should always be
appropriate for the machine. Always collect the adequate size of sample, only the amount that is
required.
