Newly Grad Online Application

Hi Mr. Santos,

Good Day!

Hope you are doing fine and well during this season.

Please be advised that based on our evaluation, your name has been included in the list of
candidates for graduation as of 2nd Semester SY 2019-2020. With that, please be informed that the
Office of the Registrar, University of the East- Caloocan campus will start to accept application and
issue credentials to graduating students as per CHED Advisory 7 starting May 20, 2020. To facilitate
the processing of your academic records (Transcript and Diploma) you can send your request by
sending an email to drrm.cal@ue.edu.ph providing the following attachments and requirements:

a. Duly accomplished application form with an electronic signature. (Please see the attached file for
the application form and make sure to attach your e-signature similar to your ID's signature.)

b. Photo of your UE ID/School ID (front and back). If you don't have your school ID with you, you can
also send at least two (2) government valid issued ID's.

c. Your selfie with your ID (Make sure both your face and ID are clear on the photo- see attached file
for reference).

d. A photo or a scanned copy of your Registration Card with Graduation fee and Alumni Fee.

e. Please be informed that the default photo to be used for your transcript of records is your School ID
Photo but if you wish to change your picture you may also send a 2x2 picture with white background
or you may ask/contact Lyric studio at lepantolyricstudio@gmail.com/544-7777 if you have already
taken your graduation pictorial.
