Digital Signature Certificate (DSC) – Org Type

Documents for Corporates / Private Limited / Public LimitedOrganization
1) DSC Subscription form
a) Completely filled.
b) Applicant to sign across his/ her photo affixed on subscription form.
c) Applicant’s signature in Section 3.
d) Section 4 – signed & stamped by any other director, (self-authorization by applicant is not acceptable).
PS: - ensure that spelling of Applicant’s name and Organization name should match with provided proofs.

2) Photo ID proof of Applicant

a) Applicant’s PAN card / Aadhar Card– (Any One- Self-attested).
b) Driving License / Passport copy / ID card issued by State Govt. OR Central Govt. Department (Any One - Self-
attested) in case PAN card not submitted.
c) Above to be attested by Bank Manager / Post Master / Gazzetted officer

3) Organization Documents.
a) Copy of Company PAN card +
b) Copy of Certificate of Incorporation +
c) Copy of Bank Statement (attested by Bank Manager OR Gazzetted officer)+
d) Board Resolution for authorizing the applicant to take DSC on behalf of Organization – signed & stamped by
any other Director (self-authorization by applicant is not acceptable).

4) Photo ID proof of Authorizing person

a) Copy of List of Directors. Certifying Authority (CA) will cross verify such details in MCA website. – (Self-attested).
b) If Authorized signatory is not a director, Board Resolution OR Power of Attorney shall be enclosed.
c) Government issued ID card (PAN, Voter ID, Passport or Driving License) or organizational ID card of Authorized
signatory – (Any One - Self-attested)

5) Attestation for Documents

a) For applying DSC, one ID proof and one address proof of Organization such as Bank Statement is also to be
attested by Bank Manager / Post Master / Gazzetted officer.
b) For organization documents – other Org documents need to signed & stamped by any other director whose
signature is present in Section 4 of the application form.

Please Note –
a) Video Verification Process:-
After the documents are sent to us, they would get an email with Video URL for Video verification where they have to click &
record live video of 25 seconds speaking the details given on Video URL page.

b) Phone Verification Process:-

For phone verification after the enrollment user will get the SMS, user has to forward that SMS to Sify numbers OR he / she
can make the call on Sify toll free number 18004192929 press 8.
