            I, ARTURO H. MAZON, of legal age, single, Filipino and with residence address at 86C
Canaoay, City of San Fernando, La Union after having been duly sworn to in accordance with
law, hereby depose and state:

1. That I am the owner of ARGEALIS CONSTRUCTION with business address at 86 C

Canaoay, City of San Fernando, La Union;
2. That the said business was registered under the Department of Trade Industry on April
17, 2017 and valid until April 17, 2022;
3. That on March 21,2020 to _________________ the said business is not operating;
4. That for this reason there was no income from the said business for the month;
5. That I am executing this affidavit to attest to the truth of the foregoing statements to
inform the proper authorities concerned, and for whatever legal purpose this may serve.
