effective 1st of September 2019.

I have decided to pursue another opportunity in a different career field for now. I have been very

I, Jhomar D. Marzan, authorizes Ms. Ma. Magnolia S. Garcia, to claim my pulled out things in
the male locker no. 71 at Discovery Primea for the reason that I could not come over because I
am currently employed at the province since the floating schedule started.

I asked her to pick up my things as well as to process my clearance at the hotel, if possible. I am
not able to file a leave at work to travel and process my documents. I have given her a copy of
my scanned UMID, Company ID and Passport to support this letter if you may need valid IDs.
