PURPOSE As an additional measure to contain the ongoing corona virus (COVID-19) outbreak in Mali, and although it is strongly advised that employees refrain from travelling outside the Sadiola Mine district, this policy is being introduced to: 1.1. Provide guidance with regards to employees who wish to apply for annual leave which is due or any other break which may become due in terms of relent company policies; 1.2, Provide guidelines for such employees to follow with regards to application of fuch leave or break, as well as social distancing, wearing of face masks, hand hygiene and completion of travel declarations if the employee had to travel duc to a family or medical emergency, 2, POLICY AND SCOPE OF APPLICATION (Eligibility) ‘The present policy is based on the following legal and regulatory requirements: * Various Goverment Decrees conceming the spread of the COVID-19 Corona Virus, as published from time to time; * Gollective Agreement of Mining, Geological, Hydro-geological Enterprises and Companies in Mali; * SEMOS SA Intemal rules and regulations; * SEMOS SA Legal Register. * Outcome of Risks assessment to mitigate or bring to tolerable level Covid 19 outbreak impact on Sadiola operations This policy will be applicable to all SEMOS SA employees who may be eligible for annual leave or any other type of break and who wish to travel for critical personal eeTpeses and must be read in conjunction with the following ‘short-term’ specific policies on: * Business travel during COVID-19 pandemic; Medical reporting and treatment; General hygiene; Social distancing: Wearing of masks; Self-isolation. Contractors are stronaly advised to adhere to this policy, 3. PROCEDURE 3.1. The Head of Department will approve the application for leave or break. 3.2. As part of the application process, the employee will be required to indicate if he/she will travel outside of the Sadiola district and if so, provide the reason/s for the planned travel as well as all the destinations the employee envisage to travel to or through. The purpose of the interview te to establish ifthe planned travel is justified given the current spread of the COVID, 3.4. If, after the interview, the HOD agrees that the planned personal travel is critical, the ‘Special Personal Travel Request during COVID-19' (Annexure B) needs to be completed and circulated to the various approval authorities 3.5. The employee must sign acknowledgement that he/she has been informed of all required protocols and that he/she agree to commit to strictly follow these protocols. (Annexure B) 3.6. Upon return to the mine, the employee/s who undertook the personal travel, will report to the SEMOS S.A. Clinic with his/her fully completed ‘Travel Declaration Form’ (Annexure C) 3.7. Depending on the medical circumstance of the employee on his/her return from the leave or break, the employee may be screened by the Clinic through testing, daily follow-up telephone calls, or the employee may be requested to sai isolate for a period of fourteen (14) days. 4. APPLICATION OF POLICY 4.1. All Heads of Department are accountable for the urgent application of this policy and procedure at the department level. 4.2. The Senior Manager HR & Administration is the custodian of this policy mine wide, with direct support from the Acting General Manager, as well as secondary support from all HOD's. 4.3. This policy is effective from its date of signature.
