Dear Beth:

Attached please find referrals for the patients I was never able to connect with. I'm not sure if you want
Amy or Sue to follow up with them, but I made at least three calls to each, and they never responded.

I have contacted or made final visits with all the patients I had on caseload. I gave Carla the blue notes
to be filed in their records. I gave the patients info. Re: outpatients counseling resources, including
Amy and Sue. Most have transportation issues and are really looking for home visiting, so hopefully
they will be able to connect with one of the few agencies that does provide this.

As you can see from these referrals, most were never logged in by Stephanie, as the majority of
referrals I received came directly to me. I am disappointed that you never discussed with me your
concerns about the number of referrals I received, as maybe there could have been a better
understanding of this. In a 10 hour work week, I found the most patients I was able to manage
effectively was between 10 and 12. (To be honest, there were many times I made, or received phone
calls on the days I didn't work for the Health Center). As I stated to you on the phone, prior to my
vacation I received a lot of referrals from Dr. Coury, and had to tell her to hold back on some, as I knew
I wouldn't be able to get to them all.

In the end, I am appreciative of the opportunity I was given to work at the health center, and when I
think about the past seven years, I am very grateful for the experience, and proud of the work I
accomplished with my patients. In saying this, when notified of the elimination of my position, I feel
disappointed I received a telephone call, and was not given the respect of a meeting, which I feel would
have been much more appropriate.
