1.0 INTRODUCTION
Pendahuluan

This standard instruction shall apply throughout the organization and shall cover all types of activities,
which involved subcontractor, visitor and public in REKAYASA.
Instruksi standard ini akan di terapkan melalui organisasi dan akan meliputi setiap jenis kegiatan,
termasuk subkontraktor, tamu dan umum dilingkungan Rekayasa.

2.0 PURPOSE
Tjujuan
To ensure that all subcontractor, visitors and public conform to HSE Policy and Procedures of
REKAYASA.
Untuk meyakinkan bahwa seluruh subkontraktor, tamu, dan umum mengetahui akan kebijakan dalam
hal
Keselamatan Kerja dan Peraturannya di Rekayasa.

To ensure that the premises, access to the contractor and plant and substances used by them are safe
and free from risks to safety, health and environment.
Untuk meyakinkan bahwa lokasi, jalan dan proyek site aman dan bebas dari risiko bahaya atas
Keselamatan, Kesehatan dan lingkungan, kepada kontraktor.

Subcontractors who are self-employed carry the same responsibilities as an employer to make proper
provision for health, safety and welfare during their activities on site.
Subkontraktor yg mempunyai karyawan sendiri melaksanakan tanggung jawab yg sama sebagai
karyawan untuk membuat kondisi yang baik untuk kesehatan, keselamatan selama melaksanakan
aktifitas di proyek.

To make sure that the visitor and public is not put at risk by any work carry out on the site.
Meyakinkan bahwa semua tamu dan umum tidak membuat suatu kegiatan yg menimbulkan risiko.

3.0 RESPONSIBILITY
Tanggung Jawab.

The HSE Committee shall be responsible for the content of this working instruction.
Panitia K-3 bertanggung jawab atas isi daripada instruksidalam pekerjaan

The HSE Committee shall be responsible to ensure all sub-contractors are registered.
Panitia K-3 bertanggung jawab untuk meyakinkan semua subkontraktor sudah terdaftar.

The HSE Committee / HSE Manager shall be responsible to ensure all sub-contractor follows
REKAYASA HSE requirements.
Paniti K-3 dan HSE Manager bertanggung jawab untuk meyakinkan semua subkontraktor
mengikuti/mentaati ketentuan-2 HSE Rekayasa.

The HSE Committee / HSE Manager concerned are responsible to ensure that the subcontractor, visitor
and public follow this procedure within the site.
Panitia K-3 / HSE Manager bertanggung jawab untuk meyakinkan bahwa subkontraktor, tanu dan
umum mentaati prosedur selama di lokasi proyek
.
HSE Manager / HSE Officer shall advice on the legal and other requirements.
HSE Manager/HSE Officer akan memberi saran dalam ketentuan hokum dan ketentuan lainnya.

All subcontractor, visitors and public concerned are responsible to comply with this working
instruction.
Setiap subkontraktor, tamu dan umum bertanggung jawab untuk menyesuaikan dengan instruksi
pekerjaan.

4.0 WORKING INSTRUCTION

Instruksi dalam melaksanakan pekerjaan

4.1 Subcontractor

Identification of Suitable Sub-contractors
Indentitas yang benar dari Subkontraktor.

The Project Manager shall ensure that all the sub-contractors selected must have knowledge of
HSE issues.
Project manager akan meyakinkan bahwa semua subkontraktor yang sudah diseleksi, harus
atau mempunyai pengetahuan tentang HSE.

All the sub-contractors who perform work in REKAYASA must be briefed by the Project
Managers / HSE Manager concerned pertaining to the HSE requirements in REKAYASA.
Setiap subkontraktor yang akan bekerja dilingkungan proyek Rekayasa harus
mendapatbriefing
dari project manager/HSE manager dalam hal HSE yang menjadi ketentuan di Rekayasa.

List of Registered Sub-contractor shall be updated by HSE Manager and must be submitted
to
HSE Committee.
Daftar Subkontraktor akan selalu diperbaharui oleh HSE Manager dan harus diserahkan
kepada Panitia K-3.

Induction Course for Construction Workers

Induksi/orientasi untuk pekerja proyek.

All construction workers must attend Induction Course for Construction Workers.
Semua pekerja harus hadir pada induksi atau orientasi program.

Awareness Training for Sub-contractors

Pelatihan dalam hal HSE untuk subkontraktor

Awareness Training for contractors shall be conducted by HSE Committee / HSE Manager
external consultant to make them understand the importance of conforming to REKAYASA’s
HSE Management System.
Pelatihan HSE untuk subkontraktor akan dilaksanakan oleh Panitia K-3 / HSE Manager dan
pelatih dari luar untuk membuat semuanya mengerti dan memahami akan pentingnya HSE
sistim di Rekayasa.

The subjects that will be covered as per Training, Awareness and Competence.
Subjek akan meliputi, Pelatihan, Kepedulian dan Keakhlian.

Accreditation
Akreditasi

Sub-contractor who provide laboratory analytical services shall furnish current and valid
accreditation for the scope of analytical services that are provided before a contract is signed
and work commences.
Subkontraktor yang menyediakan laboratorium penelitian harus/akan menyediakan akreditasi
yg masih berlaku untuk scope pekerjaan analisa servise, dan harus disiapkan sebelum kontrak
ditanda tangani dan pekerjaan dimulai.

The copy of the accreditation certificate shall be kept as record by related site.
Salinan daripada sertifikat akreditasi akan disimpan sebagai catatan di lokasi.

Licenses, Permits and Approval

Perijinan dan persetujuan

All scheduled waste contractors shall provide and get approval from Project Manager and
HSE Manager for the transport and disposal of scheduled wastes before a contract is signed
and work commences.
The copy of the license shall be kept as record by related site.
All licensed sub-contractors are required to comply with the conditions of their licenses.
Dalam pengangkutan Limbah, kontraktor harus menyiapkan jadwal yang disetujui oleh
Project
Manager dan HSE manager sebelum penanda tanganan kontrak dan sebelum pekerjaan
dimulai.Salinan dari lisensi harus disimpan dan direcord sebagai rekomendasi di lokasi.

Sub-contractor Audit
Pemeriksaan Subkontrktor

All subcontractors shall be subjected to an audit every three month by HSE Auditor for
competence and compliance to the HSE Policy and Procedures for REKAYASA.
Setiap subkontraktor akan diperiksa setiap 3 bulan oleh pemeriksa dari HSE untuk
mengetahui sejauh mana ybs mentaati prosedur keselamatan Kerja Rekayasa.

Sub-contractors who have failed first audit shall require correcting the problem within two
weeks.
Subkontraktor yg dinyatakan gugur pada pemeriksaan pertama akan diminta untuk
memperbaiki kesalahannya dalam waktu 2 minggu.

Sub-contractors who failed second audit consecutively shall be reported to Project Manager
and Project Manager shall be forwarded to Top Management for further action.
Subkontraktor yang dinyatakan salah pada saat pemeriksaan kedua, akan dilaporkan kepada
project manager dan project manager akan meneruskan kepada pimpinan perusahaan untuk
mendapat instruksi lebih lanjut.

The sub-contractor audit schedule shall be established and maintained by HSE Auditor who
also conducts the audit. This schedule shall be revised as and when necessary.
Jadwal pemeriksaan untuk subkontraktor akan siap dan dipelihara oleh HSE auditor yang
akan melaksanakan audit, jadwal ini akan direvisi bila diperlukan

The result of audits shall be kept as record by HSE Department, will be distributed to the
subcontractor and report to HSE Department Jakarta to follow up the improvement.
Hasil pemeriksaan (audit) akan disimpan dan dicatat oleh HSE Dept., dan akan
didistribusikan
kepada subkontraktor dfan dilaporkan ke HSE Jakarta.

Controls of Sub-contractors on Site

Pengawasan subkontraktor di lokasi

The HSE Committee concerned and HSE Manager / HSE Officer shall coordinate all aspect
of the contract especially related to HSE issues.
Paniti K-3 dan HSE Manager/Officer akan mengkoordinir setiap aspek dari kontrak
khususnya yang berakitan dengan masalah HSE.

A pre-contract commencement meeting held with the contractor and sub-contractors as

necessary, to review all HSE aspects of the work. The subcontractor should also be asked to
appoint a liaison person to ease later communication problems, which may arise.
Pertemuan sebelum pekerjaan dimulai dengan kontraktor dan subkontraktor adalah perlu,
untuk memeriksa ulang semua aspek HSE dalam pekerjaan. Subkontraktor harus menanyakan
point-2 yang dapat menyebabkan masalah dalm komunikasi.

Arrangement of regular progress meetings between all parties, where HSE is the first agenda
item.
Menyiapkan meeting progress secara regular dengan semua bagian terkait dengan agenda
pertama masalah HSE.

During the routine and non-routine activities, all HSE issues will be brought attention to the
HSE Committee and HSE Officer using the Service Check sheet / Report issued by the
relevant subcontractors.
Dalam kegiatan rutin atau non rutin, semua masalah HSE di sampaikan sebagai perhatian
kepada Panitia K-3 dan HSE officer dengan menggunakan service check sheet/report yg
dikeluarkan oleh subkontraktor.

For any accidents / incidents occurred while performing the job in REKAYASA, refer to
Accidents Reporting
Dalam kejadian kecelakaan pada saat melaksanakan pekerjaan di Rekayasa, ikuti prosedur
laporan kecelakaan

All the subcontractors must make sure that the worksite is clean and tidy all the time while
performing work in REKAYASA as per Housekeeping and Cleanliness Management.
Setiap subkontraktor harus memastikan bahwa kondisi proyek selalu dalam keadaan bersih
dan
rapih setiap saat selama pekerjaan berlangsung sesuai dengan ketentuan dalam penataan
rumah tangga dan kebersihan.

The Project Managers / Construction Manager / HSE Manager concerned should make sure
subcontractor leave the worksite clean and tidy; removing all wastes, materials, tools and
equipment.
Project manager/Construction manager/HSE manager harus memeriksa kondisi lokasi
subkontraktor sebelum meninggalkan lokasi, harus dalam keadaan bersih, rapih semua
sampah, barang, peralatan dsb. Harus sudah diangkat.

4.2 Visitors
Tamu

All visitors are must report to the security before being allowed to enter the site.
Setiap tamu yg berkunjung harus lapor dulu kepada petugas keamanan sebelum memasuki
lokasi proyek.

All visitors must sign in a logbook and be given a visitor pass.

Harus mengisi buku tamu dan memakai badge tamu.

A prominent sign will be put on the outer fence of the site, with tells the visitor that the
worksite is dangerous.
Poster and petunjuk2 keselamatan dan keamanan akan di tempatkan diluar pagar, sehingga
dapat menjelaskan kepada setiap tamu bahwa lokasi proyek adalah tempat yang perlu
perhatian dalam hal Keselamatan.

All visitors will be told about the hazards that may be present at that stage of the work and
must have (or be given) appropriate PPE before they are allowed to enter the site.
Setiap tamu akan diberitahu tentang bahaya-2 yang mungkin dapat terjadi, sebelum diijinkan
masuk lokasi harus dilengkapi dulu dengan alat pelindung diri.

The person in charge of the Site Office / Security must make sure that visitor is looked after
by
someone who is a representative of the principal employer.
Petugan dikantor proyek dan atau petugas keamanan harus meyakinkan bahwa setiap tamu
ada
seseorang yang merupakan karyawan yang dikunjungi atau yg mewakili.

4.3 Site security:

Keamanan Lokasi Proyek.

The site must be fenced to prevent unauthorised people from coming onto it. Fencing and
hoardings used must be sufficient to stop young children from entering the site.
Lokasi proyek harus dip agar untuk mencegah masuknya orang yang tidak mempunyai ijin
atau bukan karyawan proyek. Pagar yang dipasang harus dapat mencegah masulnya anak-2
kecil.

Material going into and out of the site must be checked by the guard on duty.
Setiap material yang masuk maupun yang keluar harus diperiksa oleh petugas keamanan.

During periods when there is no one on site, it must be properly locked and secured.
Selama di lokasi tidak ada karyawan atau kosong, semua pintu dan pintu masuk proyek harus
tertutup dan dikunci.

Where the worksite cannot be surrounded by a fence, holes or excavations must be safely
covered or fenced; materials stacked safely and plant turned off and locked out.
Bila lokasi proyek tidak dapat dipagar seluruhnya, setiap lubang atau galian harus ditutup
atau
Dipagar. Material harus ditumpuk dengan aman.

4.4 Public
Umum

Where any hazardous activities are carried out in or near public areas, steps should be taken to
make sure that the public are not put in any danger. This includes the use of barriers, signs or
the
screening-off of the hazardous area.
Bila ada kegiatan yang berbahaya dilaksanakan di loaksi atau berdekatan dengan daerah
umum, harus dilakukan tindakan pengamanan agar umum atau masyarakat tidak berada
dalam kondisi berbahaya, termasuk pemasangan tirai/penghalang, poster atau penutupan
lokasi bebahaya.

Poster peringatan akan dipasang diluar pagar, sebagai informasi lokasi berbahaya.
