Procedure: Intubation
Physician(s):
Indication: Acute hypercapnic/hypoxic respiratory failure / inability to protect airway
Sedation/Neuromuscular blocker:
The patient was prepared in the usual fashion, and a [size] endotrachial tube was placed under direct visualization, then taped at [???] inches at
the teeth. Exam revealed bilateral breath sounds and good chest rise without air sounds in the abdomen. End-tidal CO 2 monitor was also used to
confirm tracheal placement. The patient was started on mechanical ventilation, and chest x-ray was ordered to assess for endotracheal tube
placement.
Complications: None
