Sir/Madam:

Please be informed that you that based on our records and our inspection, you have
made an illegal mechanical installation in violation of the provisions of the latest Philippine
Mechanical Code and the “National Building Code of the Philippines” (PD 1096) and its
Implementing Rules and Regulations as per attached Mechanical Inspection Report.
You are hereby directed to remove all illegal installations on your building/structure
and/or premises within two (2) days and secure a mechanical permit before installation.
Failure to comply with this Notice shall cause this Office to institute administrative
sanctions and/or institute legal action against you.
