Select 4 dimensions of wellness from the activity on Assessing Your Life Balance and create
your wellness plan. The table prepared on page 19 of your module is just a guide. You are free
to be creative in presenting your wellness plan provided you adhere to the criteria provided:
clarity of statements guided by SMART, coherence, and completeness.

WELLNESS INVENTORY ESSAY


Evaluating your health and wellness status critically, draw a sensible conclusion applying your
wellness inventory based on the different tests performed beforehand, such as fitness test,
assessing your life balance or wellness wheel, and PAR-Q. Here are some guide questions that
might help you. (30 points)
1. Which aspects of which dimensions are you ready and willing to improve or develop?
2. What are/would be the benefits of working on this behavior?
3. What could get in the way of achieving your goal?
4. How can you reward yourself for achieving your desired goal?
5. What can help you achieve your goal?
6. On what date will you start?
7. What’s the point of doing all these? What’s in it for you?
