Letter of ethical clearance

ETHICAL CLEARANCE

The undersigned below, the chairman of the commission for ethics of health research and
research institution and service to the society of Jambi University, after conducting the
discussion and assessments, hereby has decided the protocol of the research that concerns:

This research is approved for implementation. The agreement is valid from the date of
determination until the deadline for conducting the research as stated in the protocol.

At the end of the research, the research implementation report must be submitted to the Jambi
University Health Ethics Commission. If there is a change in protocol and / or an extension of
the research, it must re-apply for a research ethics review (protocol amendment).
