AFFIDAVIT OF OWN DAMAGE

I, ALDRIN T. OLBES, of legal age, married, and a resident of Purok 5,

Immaculate Conception, Lubogan, Toril, Davao City, after having been sworn to in
accordance with law, do hereby depose and state that:

1. I am duly licensed driver in accordance with pertinent Land Transportation

laws, rules and regulations, and was issued a corresponding Professional
Driver’s License with License No. L02-96-112398 valid until August 25,
2024;

2. I am the registered owner of a Nissan Navarra Pick-up with Plate No.

LAD5168, the Certificate of Registration with No. 338139723 is attached
hereto as Annex A;

3. Sometime on February 2, 2021, I parked the said motor vehicle along

Lubogan, Toril Highway. When I returned to get my vehicle at around
