Commitment Statement Contract
1. | understand that this Physical Education course requires me to participate in 30 minutes of
Physical activity, at least five times per week, for the duration of the course. | will be assigned
Course Workouts for the first two weeks, after which | will design my own Custorn Workouts
according to my fitness goals and course guidelines.
2.1 will accurately document my workouts, have a parent or guardian monitor and sign off after
every workout, and turn in a Fitness Log to my teacher once a week.
3. My guardians and | will take complete responsibility for my safety, proper insurance, and
personal limitations and conditions. | do not hold Apex Learning responsible in the case of any
injury or illness resulting from a workout.
4. | understand that this course is based on my honesty and integrity in documenting my
participation in physical activity. If it is proven that | did not complete the physical component of
this course, | understand that | will not receive credit.
5. | understand that the physical component of this course is very important, and will account for
a significant part of my grade in the course.
