It is certified that mentioned employee belongs to (LCC Pakistan Pvt Ltd) and has provided insurance by the (LCC
Pakistan Pvt Ltd.

The Insurance Certificate is subject to the following Special Conditions:

(i) This Certificate is only valid for Huawei ISDP Software and is being distributed to all the eligible employees
of LCC Pakistan (Pvt.) Ltd (“the Company”) on the advice of Huawei.

(ii) This Certificate is only valid and effective in case the Employee is on active duty and performing activities for
the Company.

(iii) This Certificate will automatically become ineffective and unenforceable for the duration the Employee
remains “On-Bench” as per Company policy, i.e. the Employee is not on active duty with the Company.

(iv) This Certificate is only valid and effective in case the main Insurance Agreement of the Company with the
insurance company (NJI) is valid and effective. In case the said Insurance Agreement expires or is terminated,
this Certificate will automatically become invalid and ineffective.

(v) In case the Employee leaves the Company, or absents himself/herself without notice for a period of more
than three (3) days, this Certificate will automatically come to an end/non-effective and will be deemed to
have been cancelled/withdrawn from the date of his absence.
