Confined Space Risk Assessment Guide

This document discusses risk assessment and hazard identification as required parts of occupational safety and health (OSH) management systems. It defines risk assessment as a tool to help with decision making, not an end in itself, and explains that the risk priority number it generates can indicate the urgency of risk control based on an organization's risk tolerance criteria. The document emphasizes that risk assessment is just one part of effective risk management and that simply doing many assessments will not make a problem safer on its own without implementing other risk management processes like policies, enforcement, and reviews. It provides an example of identifying a hazard of riding a bike while distracted and assessing its potential consequence and likelihood of risk.

OSH MANAGEMENT
The law requires employers to implement OSHA.
Basically, by subscribing to an OSH Management
System such as MS 1722 or OHSAS 18000 you are
already implementing a major part of OSHA.
Implementation of the Management Systems require
Hazard Identification, Risk Assessment, and Risk
Control or Determining Controls (OHSAS)
Hazard Identification and Risk Control will be
discussed here, with an intro on Risk Assessment.

RISK ASSESSMENT
First question : What is Risk Assessment?
Risk Assessment is just a tool to assist us in decision making! It is not
an end in itself. The risk assessment will help to determine the priority
for action. Urgency of control can be indicated by the Risk Priority
Number (RPN). The organisation’s Risk Tolerance Criteria will
determine the Decision for Action (DFA).
However the output depends very much on the input, or else it will be
GIGO! Garbage in, garbage out.
Remember : Risk Assessment is just one cog on the wheel of Risk
Management. Doing hundreds of risk assessments on the same
problem will not make it any safer; safety happens when the other
cogs (policy, implementation, enforcement, review, etc) are in place
